Road failure is a most problem of developing countries in view of high cost of maintenance and reconstruction. Types of road failure that existing on the study area are pothole, cracking, bleeding and etc. The possibly cause of road failure are overloading vehicle, insufficient material mixed, maintenance and environment. This road failure can causes traffic jam and accident. In this study, it can be identify the types of road failure and identify the causes of road failure in Jalan Bemban – Serkam (M109). As a result, it can make a strip map to know the failure along the road. Then, the calculation about the percentage of road damage and road not damage can detect the specific factor of damage if it because of heavy vehicle. In addition, the interview with the engineer and assistant engineer from JKR, Jasin person in charge on Jalan Bemban – Serkam (M109) can know more detail about that road. Lastly, interview with three residences of Jalan Bemban – Serkam (M109) to know the residences opinion about the road failure.
